If you need to resign from your job due to illness and you need to do this with immediate effect then you will have thought this through carefully and be left with very little options.
Usually in this kind of scenario you will have discussed carefully with your Manger and they will be aware of the situation you are in so it won’t come as a total surprise or shock to them. Therefore writing this letter should at this stage be more of a formality and to confirm your previous discussions.
If however you have not spoken to them before hand then this letter is clearly going to be more difficult to write as they will not be aware of any of the information. I find in this situation that it be highly unlikely.

I am sorry that in this situation that I am unable to give you any notice to leave COMPANY NAME but I am very grateful for all the opportunities you have given me over X years/months. I feel that COMPANY NAME has a very good future and I am sorry not to be able to be part of that.
